Permanent Housing Program Case Manager

FTE - Exempt
The AIRS Permanent Housing Program provides permanent housing and supportive services to formerly homeless, HIV+ individuals, with the intent of stabilizing them and promoting self-sufficiency. This Case Manager is responsible for helping residents create and implement case plans, and for coordinating referrals to other service providers as needed and have a background in mental health services. The Case Manager reports to the Director of Programs.

Responsibilities
  1. Provide case management services for up to 25 residents who are housed in the Permanent Housing Program.
  2. Perform a written needs assessment in conjunction with resident to identify personal goals and develop an individual service plan.
  3. Meets face-to-face with residents at least monthly to continue addressing personal goals and service plan.
  4. Coordinate outside services with legal, financial, and other necessary services on behalf of the resident.
  5. Encourage residents to participate in AIRS’ services and activities, including enrichment activities and an in-house chemical dependency counselor.
  6. Participate in regular staff meetings and case conferences.
  7. Maintain up-to-date records for each resident and provide monthly reporting information.
  8. Attend community meetings as appropriate.
  9. May lead resident meetings or specialized support groups.
  10. Other duties as assigned by the Director of Programs
Qualifications
The Case Manager should have a bachelor’s degree in social work or related field, and a minimum of 2 years experience working with people in a medical or mental health environment. Additional experience with people as living with HIV/AIDS, homelessness, and/or substance abuse is preferred. Ideal candidate must have good written and verbal communication skills, as well as computer literacy. Effective time management skills are a must. Prior directly related experience may be substituted for education. Must have access to reliable transportation.

Life Skills Coordinator—Youth Services

0.5 FTE Position
The Life Skills Coordinator-Youth Services has primary responsibility for implementation of all aspects of the Independent Living Program for youth enrolled in the Transitional Living Program, (TLP) and the Permanent Housing Program, (YIP). Additional duties include coordination of volunteer and paid staff who are ILS instructors, and providing primary backup coverage for TLP in the absence of the TLP Coordinator. The Life Skills Coordinator reports directly to the TLP Coordinator.

Duties and Responsibilities
  1. To administer the life skills assessment within the established time frame to residents of both YIP and TLP.
  2. To complete an initial 30-day goal ILS Plan for each client in a timely fashion, updating quarterly.
  3. To document all services provided within the individual resident’s record; to document attendance at all events, as well as sequence completions by each residents.
  4. To teach life skills to clients on an individual basis.
  5. To coordinate and facilitate weekly social support or ILS Support Groups
  6. To provide individualized transitional services including a.) Locating full-time employment b.) Locating and moving into alternative housing, and c.) Providing transportation when necessary to job interviews or apartment hunting trips.
  7. To act as liaison with the youth’s employer and guidance counselor to monitor progress
  8. To deal with crisis situations as they arise.
  9. To coordinate volunteers, outside providers or staff who may be implements parts of the resident’s ILS Plan.
  10. To provide primary back-up coverage for TLP in the absence of the TLP Coordinator.
Position Requirements:
  1. BA in Human Service field with 2 years of case management experience with homeless or high-risk young adult minority populations; at least 1 year experience with running psychoeducational youth groups. Extra experience may substitute for college degree.
  2. Excellent organizational and management skills.

Youth Initiative Case Manager

Full-time Exempt Position
The Youth Initiative Case Manager is responsible for helping residents create and implement case plans and for making appropriate referrals to other service agencies as needed. The Case Manager functions as part of a team working with the Residential Counselor, Educational Mentor, Life Skills Coordinator and the Empire Homes Staff. The Youth Initiative Case Manager reports to the Director of Programs.

Duties and Responsibilities
  1. Provide case management services for up to 20 residents who are housed in the Permanent Housing Program.
  2. Perform a written needs assessment in conjunction with resident to identify personal goals and develop an individual service plan.
  3. Meets face-to-face with resident bi-weekly to continue addressing personal goals and service plan.
  4. Coordinate outside services with legal, financial, and other necessary services on behalf of the resident.
  5. Encourage residents to participate in AIRS’ services and activities, including enrichment activities and independent living skills groups.
  6. Participate in regular staff meetings and case conferences.
  7. Maintain up-to-date records for each resident and provide monthly reporting information.
  8. Attend community meetings as appropriate.
  9. May lead resident meetings or specialized support groups.

TLP Residential Counselor

Part-Time/ Full-Time
The TLP Residential Counselor is responsible for the supervision of assigned TLP youth, and is responsible for teaching life skills and observing and guiding youth as they practice these skills. The TLP Residential Counselor reports directly to the TLP Coordinator.

Duties and Responsibilities
  1. To be on the premises during the evening hours or weekends, in accordance with agreed-on schedule
  2. To ensure that each assigned TLP youth submits a weekly schedule outlining work and school hours.
  3. Provide guidance for TLP youth as they practice life skills, including cooking, shopping, grooming, cleaning, modern repair, and conflict resolution.
  4. To deal with crisis situations as they arise.
  5. Post a weekly schedule for maintenance of the common living areas.
  6. To submit weekly verbal or written reports on each TLP youth, and keeping the TLP Coordinator informed of any problems or difficulties.
  7. To ensure the safety of the TLP apartment space by reporting or handling all maintenance concerns to the proper authorities.
Position Requirements:
  1. Minimum of high school diploma, and 2 years of living independently.
  2. Understanding of older adolescents, with ability to communicate effectively with them.
  3. Mature individual with sound judgment to deal with multiple and changing situations.
  4. Excellent organizational and management skills.

Community Liaison Worker – Acclimation Support – Hispanic Services

The Community Liaison Worker – Acclimation Support – Hispanic Services is responsible for developing and maintaining a range of economic and educational support services in the community for clients of the HOPWA Hispanic Program that supports their movement towards English competency and living wage employment.

The position works cooperatively with the Hispanic Case Manager, Outreach Worker and community partnerships, and reports to the Director of Programs.

Duties and Responsibilities
  1. Develop and maintain regular activities within the Hispanic Provider Community that allow the development of trusting relationships with target client base, (high risk for HIV+ transmission).
  2. Has primary responsibility for development of appropriate ESOL services with community partner that target vocational language competency.
  3. Reinforces classroom learning on a 1:1 basis or small group social tasks.
  4. Researches best practices in the area of vocationally appropriate job training and language training for Hispanic population.
  5. Works with community vocational job coaching partner on identifying and developing a similar system/ job-training network for Hispanic clients.
  6. Develop a system of tracking progress around skill acquisition, social acclimation, and income growth. Documentation will consist of case record recording and quarterly progress reports to both AIRS and community partners.
  7. Participate in regular staff meetings and case conferences.
  8. Adhere to basic confidentiality standard for AIRS clients
  9. Other duties as assigned by the Director of Programs.
Position Requirements:
  1. Minimum BA – Education or Vocational Education preferred. Prefer some college courses in the human services or health area.
  2. Minimum of basic understand of HIV/ AIDS and STD issues, especially as it relates to the Hispanic Community in Baltimore.
  3. Must be bi-lingual, (English and Spanish), have good written and verbal communication skills, as well as computer literacy.
  4. Effective time management skills.
  5. Must have access to reliable transportation.
  6. Flexibility, creativity and enthusiasm for the project and for team work.
  7. Judgment when to seek Director of Programs’ guidance.
